EXHIBIT "D" <br />REPORTING <br />Contractor shall provide quarterly and annual progress reports by answering the questions <br />provided in this Exhibit. Please submit progress reports to the clerk at <br />healthand_humanservices@co.kittitaswa.us by the last day of the month following the <br />quarter in which services were delivered. <br />The report shall include, at a minimum: <br />A narrative of project activities during the reporting period. <br />■ A narrative of project successes during the reporting period. <br />a A narrative of project challenges during the reporting period. <br />n A narrative of progress made toward the project objectives. <br />The County retains the right to request clarification or additional information of the <br />applicant as needed to assess project progress and/or appropriate use of funds. <br />Failure to submit the required reports may result in delayed reimbursement payments, <br />denial of invoices, or contract termination. <br />Kittitas County Professional services Agreement <br />Page 20 of 20 <br />
