KITTITAS COUNTY <br />AGREEMENT FOR SERVICES <br />PHDD.OOg <br />This Agreement for Services (hereinafter "Agreement") is entered into by and between Kittitas County <br />(hereinafter "County"), a political subdivision of the State of Washington, and Kittitas County Health Network <br />(KCHN) (hereinafter "Contracto/'). <br />The purpose of this Agreement is as follows: To provide a coordinated and comprehensive state and local <br />program of services for persons with developmental disabilities in accordance with Revised Code of Washington <br />chapter 714.1,4. <br />The term of this Agreement shall be from June !'1,2025, through June 30, 2026, unless the Agreement is <br />terminated early or its term is extended as provided herein. <br />The parties' addresses and points of contact for the administration of this Agreement are as follows: <br />COUNTY CONTRACTOR <br />This Agreement includes the following, which are attached hereto and hereby incorporated by this <br />Kittitas County Public Health <br />507 N.
