ATTACHMENT "A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following activities as indicated in <br />your Performance Indicators and Evaluation Workbook for the CWU Community Clinic project, attached to this <br />Scope of Work: <br />• Provide licensed and unlicensed staff support <br />• offer mental health counseling services to youth and their families in Kittitas County <br />• Train and oversee its graduate students providing services <br />• Provide adequate facilities at CWU's Community Counseling Clinic <br />• Materials and supplies, including but not limited to surveys, books, and manuals <br />Provide and maintain a HIPAA compliant healthcare portal <br />Kittitas County Agreement for Services (rev. 5/14/25) <br />Page 3 of 32 <br />
