How does it address the objective of increasing prevention and enhancing access to care? Those responsible for the Community Clinic project are proposing to continue offering counseling services for Kittitas County residents in CWU's Community Clinic to expand access to community counseling services. Services currently include individual therapy with a Licensed Mental Health Counselor or with graduate students in related graduate programs under supervision. In addition. we will expand our Sernce to more age ranges and more clients. Our goal is that by expanding the services available we can contribute to more access to counseling services in Kittitas County. Activities: The actions or tasks you will perform to achieve your project objective(s). Detail each step you will take to reach the desired outcomes_ For successful applicants, these activities will be listed in your contract's Scope of Work. Quirnac-hr r : Communicate with community partners/referral sources/participate in community meetings. : Respond to all referrals that we get in the clinic and offet information and services. Utilize Evidence Based Practices within the Clinic_ c. n n : Utilize the suicide assessment tool to screen for suicide and utilize safety plans as indicated. Use standard assessment tools to screen tot mental health symptoms and throughout the course of treatment to help monitor outcomes. irainin� : Meet our obligation to train school psychology and mental health counseling graduate students. Expand number of youth and families served in the tunic_ Expand the age range of youth served in the clinic. outputs (Choose at least 4 outputs for this project.)
