The County Coroner may on occasion store operating supplies for autopsies at the <br />Contractor's facility. These supplies shall only be used for Coroner business. <br />EXHIBIT "B'' <br />COMPENSATION <br />The Coroner will pay the Contractor on a case-by-case basis determined by use for the <br />duration of this contract for the performance of morgue services in Kittitas County. <br />Payments to Contractor will be made in monthly according to invoices in the following <br />manner:. Payment for Facilities $300.00. Payment for Transfer (2 person) $499.00o Payment for Transfer (1 person) $349.00o lntake when Coroner Office Transports $175.00. lndigent Cremation $863.00o Transport Out of County (Autopsy) $300.00 + WA State Per Diem Rate for <br />Mileageo Transport ln County (CT Scan) $250.00 + WA State Per Diem Rate for Mileage <br />o 4s-Day-Hold Storage, Jurisdiction Held or Unable to Locate NOK $450.00 <br />EXHIBIT ''C" <br />PROOF OF INSURANCE <br />The Contractor shall secure and maintain in effect at all times during performance of the <br />Work such insurance as will protect Contractor, its Support and the Additional lnsured's <br />from all claims, losses, harm, costs, liabilities, damages and expenses arising out of <br />personal injury (including death) or property damage that may result from performance <br />of the work or this Agreement, whether such performance is by Contractor or any of its <br />Support. <br />All insurance shall be issued by companies admitted doing business in the State of <br />Washington and have a rating of A-, Class Vll or better in the most recently published <br />edition of Best's Reports unless otherwise approved by the Coroner. lf an insurer is not <br />admitted, all insurance policies and procedures for issuing the insurance policies must <br />comply with Chapter 48.15 RCW and 284-15 WAC. <br />All coverage provided by the Contractor shall be primary and non-contributory. The <br />Contractor shall provide proof of insurance for Commercial (comprehensive) General <br />Liability insurance with coverage limits not less than $2,000,000 combined single limit <br />Professional Services Agreement <br />Page 13
