Endorsement as evidence that policies providing insurance required by this Agreement <br />are in full force and effect. The following types of insurance are all required by this <br />Agreement: <br />❑ 1)Commercial General Liability Insurance <br />Thirty (30) days written notice to the Coroner of cancellation <br />of the insurance policy. <br />❑ 2) Workers' Compensation. <br />Workers' Compensation in amounts required by law. <br />❑ 3) Automobile Liability Insurance. <br />Thirty (30) days written notice to the Coroner of cancellation <br />of the insurance policy. <br />NOTE: No contract shall form until and unless a copy of the Certificate of Insurance, <br />properly completed and in the amount required, is attached hereto. <br />Professional Services Agreement <br />Page 14 <br />
