KITTITAS COUNTY PUBLIC HEALTH 2025.2027 CONSOLIDATED CONTRACT CONTRACT NUMBER: CLH320ss AMENDMENT NUMBER: 6 PURPOSE OF CHANCE: To amend this contract between the DEPARTMENT OF HEALTH heleinafter refered to as "DOH", and KITTITAS COUNTY PUBLIC HEALTH, a Local Health Jurisdiction, hereinafter refened to as "LHJ", pursuant to the ModificationsAflaivers clause, and to make necessary changes wilhin the scope ofthis contract and any subsequent amendments tlrereto.
