ATTACHMENT "A" <br />SCOPE OF WORK <br />Provide schoolto work services to eligible students in three phases including schoolto work intake and assessment <br />phase, job placement phase, and employment phase. <br />o Ensure eligibility and complete a student intake process. <br />o Work with students, student's legal guardian or representative, community rehabilitation <br />program, and DVR and establish and document student employment goals and tools needed. <br />o Complete assessment using Student Guide and/or Job Foundations report to complete the Student Guide <br />to include DVR required information. <br />o Work with the students to find and secure meaningful community employment based on goals. <br />o Work with students to stabilize and maintain meaningful employment. <br />o Complete the School to Work success story for each student <br />r Submit required documentation including student guide. <br />Kittitas County Agreement for Services (rev.5/L4/25) <br />Page3 of 22
