Exhibit B: Schoot-to-Work Student Guide Instructions The fottowing is a bitting guide for the Contractor as they work with students by fottowing the pages of the Student Guidd. As the fottowing sections are compteted, the Contractor wit[ use Exhibit F: Schoot-to-Work Bitting Checktist for Student Guide, provided to the Contractor by DVR. These instructions witt support guidance to School-to-Work contractors as they work with students who are participating in Schoot-to-Work. To begin the process of Schoot-to-Work and to reviewthe Schoot-to-Work Student Guide, schedule a Schoo[-to-Work lntake and Assessment meeting with the Student and the Student's Team. During this meeting, the Schoot-to-Work Student Guide witt be shared with the student, going through the process of Schoot-to-Work and what the student should expect in their finalyear of Transition. Bitting Instructions: 1. Bitl $3,000 for the School-to-Work Intake Meeting once the fottowing pages are complete: Team Buitding and lnformation Team Contacts, Rotes, and Responsibitities 2. Bitl $2,000 for Assessment once the fottowing pages are comptete: Assessment pages may inctude the fol,towing pages being compteted, as appticabte for the student, as each student may get to their emptoyment goat in their own way. lf a Job Foundation Report was enough for the student to set their emptoyment goat, no additionat assessment may be needed. lf the Job Foundation Report was not enough, an assessment may stitt be needed. Or, a student did not go through Job Foundation and an assessment is needed no matter what to get to an emptoyment goat. The student and the student's team wit[ work together to determine this. The main goat for this section is to get to an employment goal. and the next steps for the student: o Job Foundation Report page with yes and no questions r Job Foundation Executive Summary and Job Recommendation o Schoot-to-WorkAssessment o Job Goa[ r Next Steps and Team Actions 3. Bitt $1,000 for the Job Seeker Tools once the fottowing pages are complete: r Job Seeker Toots (both required tools are comptete and shared with the student and DVR) 4. Bitl $Z,OOO for the Job Offer once the fottowing page is complete: r Job Offer Detaits on the page titted "You were offered the job! Way to go!" 5. BilL $1,000 for Emptoyment Verification once the foltowing page is comptete: o Form is compteted on the page titted "Congratutations, you are emptoyed!" a
