Exhibit B: School-to-Work Student Guide lnstructions The fottowing is a bitting guide for the Contractor as they work with students by fottowing the pages of the Student Guide. As the fottowing sections are completed, the Contractor wit[ use Exhibit F: Schoot-to-Work Bitting Checktist for Student Guide, provided to the Contractor by DVR. These instructions wit[ support guidance to Schoot-to-Work contractors as they work with students who are participating in Schoot-to-Work. To begin the process of Schoot-to-Work and to review the Schoot-to-Work Student Guide, schedute a Schoot-to-Work lntake and Assessment meeting with the Student and the Student's Team. During this meeting, the Schoot-to-Work Student Guide witt be shared with the student, going through the process of Schoot-to-Work and what the student shoutd expect in their f inaI year of Transition. Bitting lnstructions: 1. Bitt $3,000 for the School-to-Work lntake Meeting once the fottowing pages are complete: o Team Buitding and lnformation o Team Contacts, Rotes, and Responsibitities 2. Bitt $2,000 for Assessment once the fottowing pages are complete: Assessment pages may inctude the fol.towing pages being completed, as appticabte for the student, as each student may get to their emptoyment goat in their own way. lf a Job Foundation Report was enough for the student to set their emptoyment goa[, no additionat assessment may be needed. lf the Job Foundation Report was not enough, an assessment may stitt be needed. Or, a student did not go through Job Foundation and an assessment is needed no matter what to get to an emptoyment goat. The student and the student's team wilt work together to determine this. The main goatfor this section is to get to an emptoyment goat and the next steps for the student: o Job Foundation Report page with yes and no questions o Job Foundation Executive Summary and Job Recommendation o Schoot-to-WorkAssessment o Job Goat o Next Steps and Team Actions 3. Bitt $1,000 for the Job Seeker Tools once the fottowing pages are complete: r Job Seeker Toots (both required toots are comptete and shared with the student and DVR) 4. Bitt $2,000 for the Job Offer once the foltowing page is complete: o Job Offer Detaits on the page titted "You were offered the job! Way to go!" 5. Bitt $1,000 for Emptoyment Verification once the fotlowing page is comptete: o Form is compteted on the page titted "Congratulations, you are emptoyed!"
