Kittitas County Agreement for Services (rev. 5/14/25) <br />Page 3 of 22 <br /> ATTACHMENT “A” <br />SCOPE OF WORK <br />Provide school to work services to eligible students in three phases including school to work intake and assessment <br />phase, job placement phase, and employment phase. <br />•Ensure eligibility and complete a student intake process. <br />o Work with students, student’s legal guardian or representative, community rehabilitation <br />program, and DVR and establish and document student employment goals and tools needed. <br />•Complete assessment using Student Guide and/or Job Foundations report to complete the Student Guide <br />to include DVR required information. <br />•Work with the students to find and secure meaningful community employment based on goals. <br />•Work with students to stabilize and maintain meaningful employment. <br />•Complete the School to Work success story for each student <br />•Submit required documentation including student guide.
