EXHIBIT "D" <br />REPORTING <br />Contractor shall provide quarterly and annual progress reports using the template <br />provided by the County. <br />Reports will be submitted to the clerk of the Homelessness and Affordable Housing <br />Committee (healthandhumanservies@co.kittit-), .wa,us) by the last day of the month <br />following the quarter in which services were delivered. <br />The County retains the right to request clarification or additional information of the <br />applicant as needed to assess project progress and/or appropriate use of funds. <br />Failure to submit the required reports may result in delayed reimbursement payments, <br />denial of invoices, or contract termination. <br />Kittitas County Professional Services Agreement <br />Page 20 of 20 <br />
