School-to-Work Assessme nt <br />Check if not applicable ! <br />It has been determined that a School-to-Work Assessment is needed to learn more about what <br />you would like to do for a job; this will help you figure out your job goal! Your job coach will <br />collect information about your strengths, skills, and support needed for success on the job.
