Exhibit B: Schoot-to-Work Student Guide lnstructions The fottowing is a bitting guide for the Contractor as they work with students by fottowing the pages of the Student Guide. As the fottowing sections are compteted, the Contractor witt use Exhibit F: Schoot-to-Work Bitting Checktist for Student Guide, provided to the Contractor by DVR. These instructions wit[ support guidance to Schoot-to-Work contractors as they work with students who are participating in Schoot-to-Work. To begin the process of Schoot-to-Work and to reviewthe Schoot-to-Work Student Guide, schedute a Schoo[-to-Work lntake and Assessment meeting with the Student and the Student's Team. During this meeting, the Schoot-to-Work Student Guide witt be shared with the student, going through the process of Schoot-to-Work and what the student shoutd expect in their f inat year of Transition. Bitting lnstructions: 1. Bill $3,000 for the Schoo[-to-Work lntake Meeting once the fottowing pages are complete: Team Buitding and lnformation Team Contacts, Rotes, and Responsibitities 2. Bitt $2,000 for Assessment once the fottowing pages are complete: Assessment pages may inctude the fottowing pages being compteted, as appticabte for the student, as each student may get to their employment goat in their own way. lf a Job Foundation Report was enough for the student to set their employment goat, no additionaI assessment may be needed. lf the Job Foundation Report was not enough, an assessment may stitt be needed. Or, a student did not go through Job Foundation and an assessment is needed no matter what to get to an emptoyment goat. The student and the student's team witt work together to determine this. The main goal for this section is to get to an emptoyment goat and the next steps for the student: . Job Foundation Report page with yes and no questions r Job Foundation Executive Summary and Job Recommendation . Schoot-to-Worl<Assessment r Job GoaI r Next Steps and Team Actions 3. Bitt $1,000 for the Job Seeker Toots once the fottowing pages are complete: . Job Seeker Toots (both required tools are comptete and shared with the student and DVR) 4.
