Measuring success <br /> Progress will be measured by reductions in new entries into homelessness, lower eviction rates, and higher rates of <br /> successful diversion.The county will also track data that represents the financial and social return of prevention, an <br /> essential measure for demonstrating stewardship and maintaining community support. <br /> Goal 4: Prioritize assistance based on the greatest barriers to housing stability and the greatest risk <br /> of harm. <br /> Individuals with the greatest barriers to housing stability and the highest risk of harm often face the most complex <br /> challenges, chronic health conditions, severe behavioral health needs,generational poverty, or histories of trauma. <br /> Many are also the least well served by traditional systems and may not consistently engage with available services. <br /> Ensuring that the community prioritizes assistance for these individuals requires clear definitions, strong <br /> partnerships,and shared understanding across sectors. <br /> Kittitas County recognizes that refining prioritization practices and defining"high risk"will require further community <br /> discussion and collaboration. This work must balance equity, safety, and system capacity, while responding flexibly <br /> to the changing needs of the population. It will also require shared guidance on how partners, including service <br /> providers, law enforcement, healthcare providers, schools, and businesses can effectively support and refer <br /> individuals with high needs. <br /> The communication toolkit being developed under Goal 1 and the training and learning opportunities outlined in <br /> Goal 2 will help build a more consistent community approach to responding to high-risk individuals.These tools will <br /> include practical guidance for frontline staff, community members, and local organizations on how to engage with <br /> individuals safely and appropriately. <br /> The community will pursue this goal through the following strategies: <br /> • Strengthen collaboration between non-profit, public, faith-based, and private organizations to improve <br /> identification of high-need individuals to support consistent triage and prioritization of high-need <br /> individuals. <br /> • Expand and support street outreach and proactive engagement with individuals at greatest risk of harm, <br /> with particular attention to those not connected to traditional service pathways. <br /> • Support approaches that seek to reduce harm and promote engagement, even when individuals are not <br /> ready to fully engage. <br /> Measuring success <br /> Success will be measured by improvements in housing stability and service engagement among prioritized <br /> individuals, reductions in recidivism, and increased coordination across partners in responding to high-need <br /> individuals.Our community recognizes that for this population,progress may look different for each person,reducing <br /> harm and improving connection to services are valued outcomes alongside more traditional measures such as <br /> permanent housing placement. <br /> Goal.5: Seek to house everyone in a stable setting that meets their needs. <br /> Housing is the foundation for stability,well-being, and community connection.Yet stable housing does not look the <br /> same for every person.
