- UnitedHealthcare Navigate; <br />- UnitedHealthcare Navigate Balanced; <br />- UnitedHealthcare Navigate Plus; <br />- another name Carrier may develop in the future that also includes the word "Navigate". <br />In the event Carrier discontinues using the brand name "Navigate" for this portfolio of <br />benefit contracts at some time in the future and adopts a different name for these benefit <br />contracts, Carrier will give written notice to you of that name change, and this definition <br />will continue to apply to benefit contracts marketed under that new name in the same way <br />as it previously did to benefit contracts marketed under names that included the word <br />"Navigate". <br />MEDICARE: <br />C. Medicare Advantage Benefit Contracts means benefit contracts sponsored, issued or <br />administered by a Medicare Advantage organization as part of: <br />(i) the Medicare Advantage program under Title XVIII, Part C of the Social Security <br />Act, or <br />(ii) the Medicare Advantage program together with the Prescription Drug program <br />under Title XVIII, Part C and Part D, respectively, of the Social Security Act, <br />as those program names may change from time to time. <br />D. I -SNP Medicare Advantage Benefit Contracts means UnitedHealthcare Medicare <br />Solutions Medicare Advantage Benefit Contracts that (A) are specialized Medicare <br />Advantage plans for special needs individuals (as that term is defined in 42 CFR 422.2) <br />and (B) exclusively enroll special needs individuals who are institutionalized (as that term <br />is defined in 42 CFR 422.2) as indicated by a reference to "Nursing Home" or "Nursing <br />Care" on the face of the valid identification card of any customer eligible for and enrolled <br />in those benefit contracts. <br />E. UnitedHealthcare Medicare Solutions Medicare Advantage Benefit Contracts means <br />Medicare Advantage Benefit Contracts that are offered through the UnitedHealthcare <br />Medicare Solutions business unit. Those benefit contracts will include a reference to <br />"Medicare Solutions" on the back of the valid identification card of any customer eligible <br />for and enrolled in those benefit contracts. <br />F. UnitedHealthcare Community Plan Medicare Advantage Benefit Contracts means <br />Medicare Advantage Benefit Contracts offered through the UnitedHealtheare Community <br />Plan business unit. Those Benefit Contracts will include a reference to "Medicare <br />Community Plan." <br />MEDICAID AND OTHER STATE PROGRAMS: <br />G. Medicaid Benefit Contracts means benefit contracts that offer coverage to beneficiaries <br />of a program authorized by Title XIX of the federal Social Security Act, and jointly <br />financed by the federal and state governments and administered by the state. <br />H. Children's Health Insurance Program (G°CHIP") Benefit Contracts are benefit <br />contracts under the program authorized by Title XXI of the federal Social Security Act <br />that are jointly financed by the federal and state governments and administered by the <br />state. <br />I. Washington Medicaid and CHIP Benefit Contracts means Washington Medicaid <br />Benefit Contracts and Washington CHIP Benefit Contracts that have references to <br />UHUSMGA. 03.12.
