Appendix 2 <br />Definitions, Products and Services <br />1. Customer. Individuals who are enrolled in benefit contracts insured or administered by <br />us or any participating entity are included in our use of the phrase "customer" in this agreement. <br />2. Participating entities. Participating entities have access to our agreement. A <br />participating entity is an entity obligated to a customer to provide reimbursement for covered <br />services under the customer's benefit contract and authorized by us to access your services tinder <br />our agreement. <br />3. Products and services. <br />a) We may include you in networks where your patients are enrolled in benefit contracts of the <br />types generally described below: <br />Washington Medicaid and CHIP Benefit Contracts. <br />b) However, this agreement does not apply to the following: <br />- Capitation arrangements for Medicare Advantage Benefit Contracts. (See below for <br />definition of capitation arrangements.) <br />- Capitation arrangements other than for Medicare Advantage Benefit Contracts. (See <br />below for definition of capitation arrangements.) <br />- UnitedHealtheare Community Plan Medicare Advantage Benefit Contracts. <br />- Workers' compensation benefit contracts. <br />- Washington Other Governmental Benefit Contracts. <br />- Benefit contracts for Medicare Select. <br />- Medicaid and CHIP Benefit Contracts for states other than Washington. <br />- TRICARE Benefit Contracts. <br />- Unitedflealtheare Medicare Solutions Medicare Advantage Benefit Contracts. <br />- I -SNP Medicare Advantage Benefit Contracts. <br />- Washington Benefit Contracts for the Uninsured. <br />- Benefit contracts where individuals are offered a network of participating physicians and <br />other health care professionals and must select a primary care physician. Such benefit <br />contracts may or may not include an out -of -network benefit. <br />- Benefit contracts where individuals are offered a network of participating physicians and <br />other health care providers but are not required to select a primary care physician. Such <br />benefit contract may or may not include and out -of -network benefit. <br />- Benefit contracts where individuals are not offered a network of participating physicians <br />and other health care professionals from which they may receive covered services. <br />This agreement does not supersede any existing agreements between the parties and their <br />affiliates that are not identified shove in section 3a of this Appendix.
