Special Terms and Conditions <br />I. "Job Foundation" — The Job Foundation process is intended to engage students earlier in targeted <br />employment planning and connection; increase partnerships with school staff to complete student's <br />Job Foundation report with actionable next steps; and increase the number of students completing <br />transition programs with a job or secondary education connection. <br />(1) The Job Foundation process and comprehensive report may replace the need for a <br />School -to -Work Assessment. To do so, the Job Foundation Report should include all <br />information necessary for the DVR Counselor to complete the vocational assessment. <br />(2) Additional assessment may be required and determined by the Student's Team should <br />the Job Foundation report not provide sufficient information to establish an employment <br />goal for the Student. <br />m. "Job Placement" means locating, securing, and placing a Student into competitive integrated <br />employment that is mutually agreed upon by the DVR Counselor, the Contractor, and the Student <br />or the Student's Team. <br />n. "Job Stabilization" means the Student, the employer, the DVR Counselor and the Contractor <br />mutually agree that a Student placed in a Supported Employment position has demonstrated and <br />maintained satisfactory on-the-job performance and has the quantity and type of long-term <br />employment supports available from DVR extended services or other long term support providers <br />that are needed to maintain satisfactory on-the-job performance. <br />o. "Long Term Services" means ongoing support services provided by an entity other than DVR after <br />the Student obtains employment and exits DVR School -to -Work services. <br />p. "Monthly Progress Report" means progress reports that are to be filled out on a monthly basis and <br />shared with the Student's Team to demonstrate progress towards employment and summarize <br />monthly work being done with and on behalf of the Student. <br />q. "School -to -Work" means a seamless transition for students with intellectual/developmental <br />disabilities from school to adulthood through employment services and connecting students to the <br />necessary resources for success. <br />r. "School to Work Assessment" means <br />(1) Identify a Student's unique work interests, abilities, and any competitive employment <br />barriers related to communication, mobility, work skills, work tolerance, self -direction <br />(cognition and learning), and interpersonal attitudes, skills, behavior, or self -care, etc.; <br />and <br />(2) Identify the nature and extent of support(s) and accommodations needed for the <br />Student to obtain and maintain competitive integrated employment. <br />s. "Service Delivery Outcome Plan (SDOP)" is a written plan jointly developed by the Student, DVR, <br />DVR Counselor, and CRP representative for obtaining School -to -Work services. The details of the <br />agreement are tailored to the unique needs of the Student and documented. <br />t. "Student" means a student with an intellectual/developmental disability age 20 to 22. <br />u. "Student Guide" means a guide designed for Students and/or parent(s)/guardians to house School - <br />to -Work related documentation, information, assessments, activity logs, and notes.
