Special Terms and Conditions <br />(a) If the Student received the job offer, Contractor must provide details of the job offer including <br />the following: <br />i. The Student's Employment Goal; <br />ii. Job Title; <br />iii. Start Date; <br />iv. Job Duties; <br />v. Expected Weekly Hours; <br />vi. Starting Wage; <br />vii. Location; <br />viii. Transportation Consideration; <br />ix. Benefits Package; and <br />x. Potential Training Services and Date/Additional Info. <br />(b) If the Student rejected the job offer, Contractor must provide the reason(s) why the Student <br />rejected the job offer. Details must include how the job did not meet their individualized <br />employment goal, as well as the criteria in (2)(a). <br />c. Employment Phase. The intent of this phase is to verify, stabilize, and maintain meaningful <br />community employment. <br />(1) Employment Verification <br />(a) Contractor shall submit the following method of verification of employment after Student's <br />first day of employment: <br />i. The Employment Verification page of the Student Guide. <br />(2) Intensive Training Services <br />(a) Intensive job training expectations shall be outlined by the Student's Team. The Contractor <br />shall provide individualized, one-on-one job skills training and support at the supported <br />employment job site that will enable a Student to: <br />i. Achieve job stabilization in on-the-job performance, with job supports and services, and <br />a fading plan; <br />ii. Meet the employer's expected level of work productivity; and <br />iii. Transition to Extended Services as needed and Long -Term Services. <br />(3) Stabilization <br />(a) Stabilization is achieved when the Student's Team and their employer agree that the <br />Student has demonstrated and maintained satisfactory on-the-job performance and has <br />DSHS Central Contract Services <br />6017CF County Program Agreement (10-31-2017) Page 8 <br />
