"Wellpolnt Medicaid Rate(s/Fee Sclredule(slMethodologles" means the Wellpoint Medlcald Rate(s/Fee Schedule(s)/ <br />in effect on the date of service for the provlder type(s/servlce(s) ldentified herein for the ipplicable Medichid <br />Program(s). <br />"CMS Outpatient Prospective Payment System" ('OPPS") shalt have the meanlng set forth in Medicare law and CMS <br />regulations and guidance. <br />"Medical Care Management Rate" means the amount paid by Wellpoint to Provider on a per member per month basls <br />for facllltation of collaborative programs meant to manage medicallsocial/mental health condltlons moie effeclively. <br />'Medicare Fee Schedule" mean$ the applicable Medicare Fee Schedule for tho provlder type(s) identifled herein, <br />including payment converslon factor, where applicable, and in effect on the date of the service is initiateO to Members, <br />Medicans Fee Schedule and/or rate changes will be applled on a prospective basls. <br />"Medicare LUPA National Base Rate' means the Medicare LUPA ("Low Utilization Payment Adjustment") National <br />Base rate ln effect as of the date of seruice for the market(s) and program(s) covored by the Agreement at thb time the <br />Covered Services are initiated lo the Member.
