SCHEDULE A:Pricing <br />"Cost" shall be defined as invoice cost. <br />Brand Medications. <br />Brand Medications witl be sold at cosi plus $3.00 per Rx <br />Gcneric Medications, <br />Gencrio Medications will be sold at cost plus $3.00 per Rx <br />Retums: <br />Return credits will be at 100% of the cost of the eligible medication. <br />The fbllowing items are either non-returnable and are not eligible for sredit: <br />r Controlled Medications (non-returnable) <br />o All eontrolled medications (Schedule II-V) need to be destroyed on site. The <br />DEA prohibits returning controlled substances to the pharmacy. ln the event a <br />conrolled rnedication is inadvertontly ieturned to Westwood Pharmacy, <br />Westwood will mail that medication back to you with directions to deshoy on <br />site. <br />. . Specialty Medications (non-returnable) <br />o Westwood Pharmacy will notify the facility prior to fillfurg any speoialty <br />nedications to nrake them aware the product is non-returnable. <br />o Westwood Pharmacy will await written permission frorn the facility to proceed <br />with filling the non-returnable specialty product. <br />r Refrigerated Products (not eligible for credit) <br />o Since Westwood Phannacy cannot vsriry the integrity of how the product has <br />been stored, refigerated products are not eligible for cre<lit. <br />. Half Tablets (not eligible for credit) <br />o If a facility requests half tabs they are not eligible for credit as Wcstwood <br />Pharmacy will not bs able to reuse that produot. <br />r Within 90 Days of the Expiration Date (not eligible for credit) <br />o If the medication is returned and it is within 90 days of the expiration date, it is <br />not eligible for credit beoause Westwood Pharmacy will not be able to reuse the <br />product. <br />. Opened tsulk ltems (not eligible for credit) <br />o If a bulk item (i.e., inhaler, crean/ointment) is returned open, this item will not be <br />eligible for credit as Westwood Pharmacy cannot verifu that they product has not <br />been used. <br />o The final decision on the integrity of thc product rvill res! upon the phannacist checking <br />the returns.I Any damaged itsns are not eligible for return' <br />4
