1 <br />2 <br />This Contract between the State of Washington Department of Social and Health Services (DSHS) and the <br />Contractor is hereby amended as follows: <br />Period of Performance. The term of the Contract shall be extended to June 30th, 2025. <br />Special Terms and Gonditions, Section 4, Consideration, is amended to de-obligate the total <br />consideration payable under this Contract by $75,700, to a new maximum of $26,000.00, including any <br />and all expenses, and shall be based on the information in Exhibit B - Estimated Bi-Annual Budget. <br />3.Exhibit B - Estimated Bi-Annual Budget has been amended to reflect the new consideration amount <br />and is attached below. <br />All other terms and conditions of this Contract remain in full force and effect. <br />DSHS Central Contract Services <br />6O24PF Contract Amendment (1 -26-2018) <br />Page 2
