This Contract between the State of Washington Department of Social and Health Services (DSHS) and the <br />Contractor is hereby amended as follows: <br />1. Period of Performance. The term of the Contract shall be extended to June 30th, 202b. <br />2. Special Terms and Conditions, Section 4, Gonsideration, is amended to de-obligate the total <br />consideration payable under this Contract by $75,700, to a new maximum of 926,0Ob.OO, including any <br />and all expenses, and shall be based on the information in Exhibit B - Estimated Bi-Annual Budget. <br />3 Exhibit B - Estimated Bi-Annual Budget has been amended to reflect the new consideration amount <br />and is attached below. <br />All other terms and conditions of this Contract remain in full force and effect. <br />DSHS Central Contract Services <br />6024PF Conlract Amendment (1 -26-201 8) <br />Page 2
