The report shatl inctude, at a minimum: o A narrative of project activities during the reporting period, incl'uding the activities outtined in Exhibit A. o A narrative of project successes during the reporting period. o A narrative of project chattenges during the reporting period. o A narrative of progress made toward the project objectives. o A data summary of the fottowing outputsr For Increasing Prevention and EnhancingAccess to Care and Services: o Number of individuals receiving or participating in evidence-based behavioral heal.th-retated-services as a resutt of the grant o Number and percentage of peopte accessing services after referral' o Number of individuats screened for behavioraI heatth-retated interventions o Number of individuats referred to crisis or other behavioraI heal.th services for suicide risk, ideation or behavior o Number of individuats screened for suicide ideation as a resutt of the grant o Number of individuats screened for trauma-rel,ated experiences as a resutt of the grant The County retains the right to request ctarification or additional information of the appticant as needed to assess project progress and/or appropriate use of funds.
