Negotiate and formulate contract with behaviorat heatth partner <br />o Formatize and strengthen retationships with organizations that are invotved in <br />providing services to program participants. <br />Whol.e Person Care Mode[ lmptementation <br />o Care Navigation and Case Management <br />o Conduct comprehensive needs assessments to identify barriers retated to <br />behavioraI heatth, substance use, housing, income, emptoyment, food <br />access, tra nsportation, socia I con nection, and i nte rpersonat safety. <br />o Make ctosed-toop referrats to and coordinate with community partners to <br />ensure ctients receive the necessary support for services external to the <br />program. <br />o Devetop personatized stabil,ity ptans with ctients, focusing on [ong-term <br />stabitity goats (housing, emptoyment, heatth) <br />o Regutarty review and adjust ctients' goats, providing tailored support and <br />ensuring accountabitity. <br />o Offer workshops or one-on-one support to devetop tife skitts for budgeting, <br />managing househotd responsibitities, and navigating sociat services. <br />o lnvotve certified peer support specialists who have tived experience to offer <br />emotionaI support and guidance. <br />Bridge Behaviora[ Heatth Support <br />o Conduct comprehensive mentat heatth assessments upon program entry. <br />o Offer regutar one-on-one counseting and behavioraI heatth services with the <br />contracted provider. <br />o Navigate and connect to [ong-term behavioraI heatth resources. <br />Housing Access and Support <br />o Rapid Housing Ptacement - ldentify and secure permanent housing for participants <br />without preconditions. <br />Kittitas County ProfessionaI Services Agreement <br />Page15ol22
