EXHIBIT "D" <br />REPORTING <br />Contractor shall provide monthly progress reports documenting clients served, trends, <br />challenges and successes using the reporting form provided by Kittitas County. <br />Reports will be submitted to the clerk of the Homelessness and Affordable Housing <br />Committee at heal_t_h_andhumanserv_ices@co._kittitas.wa.us by the by the last day of the <br />month following the calendar month in which services were delivered. <br />The County retains the right to request clarification or additional information of the <br />applicant as needed to assess project progress and/or appropriate use of funds. <br />Failure to submit required reports may result in delayed reimbursement payments, denial <br />of invoices, or contract termination. <br />Kittitas County Professional Services Agreement <br />Page 20 of 20 <br />
