EXHIBIT "A" <br />SCOPE OF WORK <br />Contractor will use the funding as local contribution to the larger Youth Homelessness <br />Demonstration Project funded by the United States Department of Housing and Urban <br />Development (HUD). Through this project, the Contractor agrees to serve youth and young <br />adults experiencing homelessness or at risk of homelessness in Kittitas County, ages 18- <br />24, by providing the following services based on the needs of the client: <br />■ Emergency housing <br />• Transitional housing <br />• Permanent housing <br />• Case management <br />• Connection to support services <br />• Family reunification <br />• Outreach <br />Kittitas County Professional Services Agreement <br />Page 15 of 21 <br />
