EXHIBIT ".A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following <br />activities to expand the KVH Crisis Case Manager (CCM) program hours to provide <br />coverage from 7 PM until 3 AM, 7 days a week: <br />• Hire two full-time staff to expand hours <br />• Train Crisis Case Managers in use of Columbia -Suicide Severity Rating Scale (C- <br />SSRS) and ITA law within first 30 days of employment <br />• Train Crisis Case Manager staff in the use of the DLA 20 within the first 90 days of <br />employment <br />• Screen all persons referred to a Crisi Case Manager using the Columbia -Suicide <br />Severity Rating Scale. <br />■ Screen at least 80% of persons referred to Crisi Case Manager using the Daily Living <br />Activities 20 (DLA 20) <br />• Refer all individuals with domain score of 4 or lower to address the need or skills <br />coaching <br />Kittitas County Professional Services Agreement <br />Page 15 of 20 <br />
