EXl.llBlT "D" <br />REPORTING <br />Contractor shal,l, provide quarterly and annuat progress reports by answering the questions <br />provided in this Exhibit. Ptease submit progress reports to the cterk at <br />hp,aLthandhurmanserviCes@co,kitti6s.wa,us by the Last day of the month fotlowing the <br />quarter in which services were detivered. <br />The report shatl incl.ude, at a minimum: <br />o A narrative of project activities during the reporting period, incl'uding the <br />activities outlined in Exhibit A. <br />o A narrative of project successes duringthe reporting period. <br />o A narrative of project chattenges during the reporting period. <br />o A narrative of progress made toward the project objectives- <br />o A data summary of the foll.owing outputs: <br />o Number of individuats receiving evidence-based behavioraI heatth-retated <br />services as a resutt of the grant. <br />o Number of organizational changes made to support improvement. <br />o Number of new staff hired. <br />The County retains the right to request ctarification or additionaI information of the <br />appticant as needed to assess project progress and/or appropriate use of funds. <br />Faiture to submit the required reports may result in del,ayed reimbursement payments, <br />deniaI of invoices, or contract termination. <br />Kittitas County Professional Services Agreement <br />Page20 oI 21
