EXHIBIT"A'' <br />SCOPE OF WORK <br />The Contractor shatt provide financial. veterinary assistance of up to $SOO per aid request for <br />residents of Kittitas Countywho self-attest to the fottowing: <br />1) They are unabte to pay for the needed veterinary care themsetves, and <br />2) The animat requiring care is a companion animal that supports the appticant's mental <br />heatth <br />And setf-attest to at least one of the fottowing criteria: <br />1 ) They are a youth age 17 and under, or <br />2) They are a United states veteran as defined by RCW 41.04.O07, or <br />3) They are a senior age 65 and over. <br />Additionat project activities i nctude: <br />Project Objective: tncreasing Prevention and EnhancingAccess to Care and Services <br />o Update Apptication forAssistance to inctude grant criteria <br />o Develop [anguage/ftiers advertisingthe newgrant program <br />o Share the fLiers with mental, heal,th ctinics and vet offices in Kittitas County <br />o Accept apptications for assistance & distribute funds to qual'ifying appticants <br />Kittitas County ProfessionaI Services Agreement <br />Page 15 of 19
