COIAMU NITY PROTECTI ON WAIVER <br />This waiver is for peopte who need 24-hour on-site awake staff supervision and therapies to maintain <br />their own and community safety. Services covered include: Residentiat Services, Behavior Support, <br />Therapies, Environmentat Adaptations, Skitted Nursing, Emptoyment services, Speciatized Medicat <br />Equipment and Supplies and more. <br />tNDtvtDUAL & FAM|Ly SERVTCES WATVER (tFS) <br />This waiver became effective June 1 ,2015, and offers support to clients tiving with a famity member in <br />the community. The intent of this program is to provide support and services to famity members, <br />centered on the needs of the client and the family. Services covered include the fottowing:o IFS funding levels range from 51,200 to 53,600, depending on assessed need (determined by <br />assessment). <br />r IFS services inctude Respite; Behavior Support and Consultation; Community Engagement; <br />Home and Vehicte Modifications; Peer Mentoring; Assistive Technotogy and Speech; <br />Occupationa[, and PhysicaI Therapies; Person Centered Ptan Facititation; Speciatized ctothing <br />and nutrition; skitted Nursing; and rransportation to a waiver service.r IFS services can be used in any combination, not to exceed a client's annual a[location. <br />'HILDREN'S <br />/NIENS/yE IN-HOME BEHAVTORAL SUq?ORT (CilBS) <br />This waiver combines wrap-around ptanning with positive behavior supports to empower families to <br />remain together. To be etigibte for CllBS, your chitd must be between the ages 8 through 20 years otd <br />tiving with their famity and are assessed at high or severe risk of out-of-home placement due to <br />chal[enging behaviors. Services covered are determined by the lndividuat Support Ptan with an average <br />cost of service, including respite, of $4,000 per month per ctient for any combination of the fottowing <br />services: Skitled Nursing, Nurse Detegation and Behavior Support and Consuttation; Environmental <br />Accessibitity Adaptations; Assistive Technotogy; Therapeutic Equipment and Suppties; Speciatized <br />Medicat Equipment/Suppties; Speciatized Nutrition and ctothing; Vehicte Modifications; OccupationaI <br />Therapy; Specialized Psychiatric Services; Physicat Therapy; Speech, Hearing and Language Services; <br />Staff/Family Consultation and Training; and Transportation. <br />ACCESSING WAIVER SERVICES <br />STEP 1 <br />ln order to access county funded services, individuats with devetopmentaI disabitities must be <br />determined etigibte for services through the Developmentat Disabitities Administration (DDA). The assessment <br />determines what services a person may be etigibte for. lf certain criteria are met, a request to be <br />placed on a Waiver Program can be made.
