It provides essential, prerelease services for individuals leaving incarceration. <br />Under this initiative, incarcerated individuals who are apple health eligible will receive services up to 90 <br />days prior to release. This is to ensure health and successful reentry to their community. It will include <br />case management, medications, and other community health worker assistance all covered under <br />Medicare. <br />_Amendment Summary: This Amendment is to add funding requirements to the Statement of Work, <br />Section 5. HCA Responsibilities. Waiver handing for Capacity Building and IT Infrastructure can only be used <br />for expenditures in Appendix A. Use of funding for any other purposes is not allowable.
