EXHIDEM„ REPORTING Contractor shall provide quarterly and annual progress reports by answering the questions provided in this Exhibit. Please submit progress reports to the clerk at healthandhumanserv_ices@co.kittitas.wa.us by the last day of the month following the quarter in which services were delivered. The report shall include, at a minimum: • A narrative of project activities during the reporting period, including the activities outlined in Exhibit A. • A narrative of project successes during the reporting period. ■ A narrative of project challenges during the reporting period. • A narrative of progress made toward the project objectives. ■ A data summary of the following outputs: ■ Number of individuals receiving or participating in evidence -based behavioral health -related -services as a result of the grant • Number and percentage of people accessing services after referral ■ Number of individuals screened for behavioral health -related interventions ■ Number of individuals referred to crisis or other behavioral health services for suicide risk, ideation or behavior • Number of individuals screened for suicide ideation as a result of the grant • Number of individuals screened for trauma -related experiences as a result of the grant • Number of individuals trained in prevention or behavioral health promotion ■ Number of individuals trained in trauma -informed care practices • Number of new staff hired ■ Number of organizational changes made to support improvement The County retains the right to request clarification or additional information of the applicant as needed to assess project progress and/or appropriate use of funds. Failure to submit the required reports may result in delayed reimbursement payments, denial of invoices, or contract termination. Kittitas County Professional Services Agreement Page 21 of 21
