EXHIBIT "D" Contractor shall provide quarterly and annual progress reports by answering the questions provided in this Exhibit. Please submit progress reports to the clerk at healthandhumanservices@co.kittitas.wa._u by the last day of the month following the quarter in which services were delivered. The report shall include, at a minimum: ■ A narrative of project activities during the reporting period, including the activities outlined in Exhibit A. • A narrative of project successes during the reporting period. ■ A narrative of project challenges during the reporting period. • A narrative of progress made toward the project objectives. ■ A data summary of the following outputs: o For Enhancing Systems and Reducing Stigma: ■ Number of people exposed to/number of public awareness campaigns and messages ■ Number and percentage of individuals who have demonstrated improvement in knowledge, attitudes, or beliefs related to prevention and/or promotion ■ Number of individuals trained in prevention or behavioral health promotion ■ Number of youth participants in prevention programs o For Increasing Prevention and Enhancing Access to Care and Services: ■ Number of individuals referred to crisis or other behavioral health services for suicide risk, ideation or behavior ■ Number of individuals trained in prevention or behavioral health promotion ■ Number of individuals trained in trauma -informed care practices ■ Number of youth participants in prevention services The County retains the right to request clarification or additional information of the applicant as needed to assess project progress and/or appropriate use of funds. Failure to submit the required reports may result in delayed reimbursement payments, denial of invoices, or contract termination. Kittitas County Professional Services Agreement Page 20 of 20
