EXHIBIT "D" <br />REPORTING <br />Contractor shall provide quarterly and annual progress reports by answering the questions <br />provided in this Exhibit. Please submit progress reports to the clerk at <br />healthandhumanseryices@co.kittitas.wa.us by the last day of the month following the quarter in <br />which services were delivered. <br />The report shall include, at a minimum: <br />• A narrative of project activities during the reporting period, including the activities <br />outlined in Exhibit A Statement of Work. <br />+ A narrative of project successes during the reporting period. <br />■ A narrative of project challenges during the reporting period. <br />+ A narrative of progress made toward the project objectives. <br />+ A data summary of the following outputs: <br />o For Enhancing Systems and Reducing Stigma <br />• Number of Individuals Trained <br />• Number of New Staff Hired <br />• Number of Schools Implementing Stigma -Reduction Programs <br />• Number of Individuals Demonstrating Improved Knowledge, Attitudes, or <br />Beliefs <br />o For Increasing Prevention and Enhancing Access to Care and Services <br />• Number of individuals receiving or participating in evidence -based <br />behavioral health -related -services as a result of the grant <br />• Number of individuals screened for behavioral health -related interventions <br />• Number of individuals referred to crisis or other behavioral health services <br />for suicide risk, ideation or behavior <br />• Number of individuals trained in suicide risk assessment as a result of the <br />grant <br />• Number of individuals screened for trauma -related experiences as a result <br />of the grant <br />• Number of new staff hired <br />• Number of individuals trained in prevention or behavioral health promotion <br />• Number of individuals trained in trauma -informed care practices <br />The County retains the right to request clarification or additional information of the applicant as <br />needed to assess project progress and/or appropriate use of funds. <br />Failure to submit the required reports may result in delayed reimbursement payments, denial of <br />invoices, or contract termination. <br />Kittitas County Professional Services Agreement <br />Page 19 of 19 <br />
