The employee must submit a "Shared Leave Request Form" to the Human Resources Department <br />along with medical certification from a licensed physician or health care practitioner which describes <br />the severity of the illness, injury, impairment or physical or mental condition and the expected <br />duration of the condition necessitating the leave. lf the employee is incapacitated, the employee's <br />representative may complete the request for the employee. <br />The Human Resources Director will review the request for compliance with established policy. The <br />employee and the Sheriff will be notified if the request is approved or denied. <br />Once the request for shared leave has been approved, and if the requesting employee consents, the <br />HR Department will notify fellow bargained employees of the employee's need for shared leave <br />donations. lf the requesting employee does not consent to be named in the notification, the notice <br />will be made as an "anonymous" request for shared leave donations. Details about the employee's <br />reason(s) for the need for shared leave will not be disclosed. <br />E. Eligibility for Donating Leave <br />Kittitas County employees may donate accrued leave to the Shared Leave Bank or to a specific <br />County employee who is eligible to receive shared leave under the following conditions: <br />. The employee must be a regular paid employee and covered under the shared leave <br />provision of the aforementioned collective bargaining agreement.. The employee was not intimidated, threatened, or coerced into donating accrued leave for <br />the purpose of shared leave.. At no time will the act of sharing sick leave cause the employee's own sick leave bank to fall <br />below 120 hours after the donated leave is subtracted.. Personal Holidays must be donated in full-day increments.. An employee may share a maximum of 40 hours per instance (qualifying event).. The donation must be submitted no later than one (1) month following the end of the <br />receiving employee's shared leave eligibility. <br />Donating Shared Leave <br />The employee must submit a "shared Leave Donation Form" to the Human Resources Director who <br />then reviews the request for compliance with established policy. Once approved, the HR Department <br />will notify Payroll. The Employee and the Sheriff will be notified whether the request is approved or <br />denied. <br />The donation will be kept confidential if requested. <br />G. Types of Donations <br />Accrued leave may be donated to the Shared Leave Bank in one (1) of two (2) methods: <br />1. General Donation: Donated leave may be used for any approved request. <br />2. Specific Donation: Donated leave may be used only for the specific person to whom it was <br />donated. Leave donated to a specific person will be returned to the donor if not utilized. <br />H.
