SERVICE AGREEMENT BETWEEN KITTITAS COUNTY JAIL AND COMPREHENSTVE HEAUTTICARE Amendment #3 This is an Amended contract betwee,n Comprehensive Mental Health, hereinafter refened to as COMPREHBNSM, 402 S 4$ Ave, Yakima WA 98902 and Kittitas County (County) on behalf of the Kittitas County Sheriff's Office for the be,nefit of the Kittitas County Jail hereinafter refened to as KC, 205 West 5s Ave, Suite 1, Rllensburg WA 98926. I. Purposo The purpose of this Amendment is to add an additional full time on-site Mental Health Counselot (lvftIc) to better support the inmates at KCJ. This will allow for a total of two Mental Health Counselors, and one Case Manager, II. Tbrm The term of thisAgreement will begin luly ltt,2024 and will terminate when both parties agree to term, unless sooner terminated according to the provisions herein. This conhact may continue from year-to-year upon mutual agreement between the two parties and based on mutual adjustment in rate of pny at3Yo, m. fnsurance During the term of ttris Agreement, COMPREHENSM shall maintain public liability and professional liability insurance ooverage in the minimum amount of $1,000,000 against claims or iqiuries to persons or damage to property which may atise from or in connection with the performance of the work by COMPREHENSM, its agents, representatives, o_r employees. A) B) c) D) E) General Terms and Conditions The period of the performance of this agreement shall be from the date of signature forwards until the modified or tenninated by either party upon timely notice. No altemation or variation of the term of this conhact or understandings or agreements not incorporated hereino unless made in writing between 0re parties hereto, shall be binding. In performing services under this agreement, lhe KCJ and COMPREIIENSIVE are independent contactiors, aild nothing herein is to be conshued as establishing an employer-employee relationship. The agencies agree that all work shall be performed in accordance with the highest professional standards, and consistent with KCJ policies and procedures and COMPREHENSIVE regul ations. The parties hereby agree that no person will be discriminated against on the basis of race, color, national origin, sex, or handicap or on the basis of any other protected status. This is in accordance with the Title IX of the EducationAmendments of 1972, and the Washington Law Against Discrimination, as amended. 1
