R efer to <br />the CH6 Verification of Household Eligibility ond lncome Recertification Form for allowable <br />documentation. <br />4.4.t <br />The CHG Verification of Household Etigibility ond lncome Recertification Form and housing status <br />documentation must be kept in the client file. Documentation must be dated within 30 days of <br />program entry. <br />Targeted Prevention <br />Homelessness prevention programs must prioritize households most likely to become homeless, and <br />must use either the CHG Targeted Prevention Etigibility Screening Form or other tool approved by <br />Commerce. This form must be kept in the client file. <br />lf modifying the CHG Targeted Prevention Etigibility Screening Form or using another tool, the risk <br />factors must be evidence informed. Submit the screening tool to your Commerce grant manager for <br />approval. <br />4.5 Homelessness Prevention lncome Eligi <br />The combined household income must not exceed 80 percent of area median gross income as <br />by HUD. Lead grantees can determine to target households with a lower area median income' <br />defined <br />lncome limits are based on Area Median lncome (AMl) which can be located for each county at: <br />www.hudu ser_sov (Data Sets, lncome Limits) <br />lncome is money that is paid to, or on behalf of, any household member. lncome includes the current <br />gross income (annualized) of all adult (f A years and older) household members and unearned income <br />attributable to a minor. lncome eligibility determinations are based on the household's income at <br />program entry. lncome inclusions and exclusions are listed in the Electronic Code of Federal <br />Regulations, www.ecfr.gov, Title 24 - Housing and Urban Development: Subtitle A 0-99: Part 5: Subpart <br />F: Section 5.609 Annual lncome <br />Gross Income is the amount of income earned before any deductions (such as taxes and health <br />insurance premiums) are made. <br />Current lncome is the income that the household is currently receiving. lncome recently terminated <br />should not be included. <br />4.5.1 lncome Eligibility Exemptions <br />tncome eligibility verification is never required for households receiving only flexible funding. <br />Page | 22
