To be eligible for pSH CHF, the head of household must meet homelessness and disability criteria <br />identified in the definition of chronically homeless in Section 3.3.2. <br />Disabilities are expected to be long-continuing or indefinite in duration and sustainability impedes the <br />household member's ability to live independently' <br />Disability includes: a physical, developmental, mental, or emotional impairment, including impairment <br />caused by alcohol or drug abuse, post-traumatic stress disorder, or brain injury. A person will also be <br />considered to have a disability if he or she has Acquired lmmune Deficiency Syndrome (AlDs) or any <br />conditions arising from the etiologic agent for 85 Acquired lmmune Deficiency Syndrome, including <br />infection with the Human lmmunodeficiency Virus (HlV). <br />3.8.1 Documentation of a Disability <br />Lead/subgrantees must verify and document the disability prior to program entry. Acceptable <br />documentation of the disability must include one the following: <br />./ Written verification of the disability from a professional licensed by the state to diagnose and <br />treat the disability and his or her certification that the disability is expected to be long <br />continuing or of indefinite duration and substantially impedes the individual's ability to live <br />independentlY.,/ Written verification from the Social Security Administration. <br />,/ Disability check receipt (Social Security Disability lnsurance check or Veteran Disability <br />ComPensation).,/ Other documentation approved by Commerce. <br />CHG Verificotion of Househotd Eligibility and lncome Recertification Form and all allowable disability <br />documentation must be kept in the client file. lf unable to document disability at program entry with <br />the above methods, program staff must record observation of disability. Required documentation <br />(above) must be obtained within 45 days of program enrollment' <br />3.8.2 Maintaining Homeless Status for Permanent Housing <br />While receiving Rapid Re-Housing assistance, households maintain their homeless status for purposes <br />of eligibility for other permanent housing placements. <br />Page | 20
