Attachment C: Performance Indicators and Evaluation Workbook <br /> <br />Next, list the specific actions or tasks you will perform to achieve your project goals. Detail each <br />step you will take to reach the desired outcomes. For successful applicants, these activities will <br />be listed in your contract’s Scope of Work. <br />Example: <br /> Project Planning and Coordination <br />o Develop a project plan with timelines and coordinate with professionals and community <br />centers. <br />o Design and review training materials, conduct a pilot workshop, and make adjustments <br />as needed. <br /> Outreach, Recruitment, and Workshop Implementation <br />o Create and distribute promotional materials, and recruit participants through <br />community collaborations. <br />o Host workshops with hands-on training, provide incentives, and ensure culturally <br />sensitive content. <br /> Follow-Up, Support, and Evaluation: <br />o Establish a follow-up system and support network, including a helpline and online <br />forum. <br />o Collect and analyze feedback, prepare reports, and make adjustments based on <br />evaluation results. <br /> Ongoing Community Engagement: <br />o Maintain participant relationships and promote mental health through community <br />events and partnerships. <br />o Continue awareness efforts and seek additional funding to expand the project. <br /> <br />3. Outputs (Select at least 4 outputs for this project.) <br />You may add alternative outputs in the spaces provided, but only 1 of these “other” outputs can be <br />considered for evaluation. At least 3 of the 4 must be from the existing list. <br />Outputs are the counts that will be required of successful applicants to report as part of their <br />project deliverables. To fill out the Outputs section, start by checking at least 4 boxes that best <br />fit your project. These are the main results you’ll use to track your progress. If you have <br />additional outputs to include, write them in the extra space provided.
