Eligible Recipients <br /> <br />Eligible applicants are: <br />• Nonproflt 501(c)(3) <br />• Private business entity offering relevant services in Kittitas County <br />• Governmental/public agency <br />• Tribe <br /> <br />Organizations must have a Federal Tax Identiflcation Number (also known as Employer <br />Identiflcation Number/EIN), be in good standing with Washington State and Kittitas County, and <br />not have been debarred. <br /> <br />All applicants must have established, appropriate flnancial internal controls and accounting <br />procedures to ensure proper dispersant and accounting of funds awarded. <br /> <br />Applicants must have general and professional liability insurance with <br />coverage for the activities of this grant with minimum occurrence and aggregate limits as <br />described further in this disclosure. Please see Attachment E: Proof of Insurance for more <br />information. <br /> <br /> <br />B. Application Requirements <br /> <br />A completed application packet must be submitted by 5 p.m. on Wednesday, September 25th, <br />2024. Applicants must complete all questions on the application, and upload the following <br />documents: <br /> <br />• RFP Summary Form <br />• Completed Application <br />• Budget Worksheet (Attachment B) <br />• Performance Indicators and Evaluation Workbook (Attachment C) <br />• Letters of Support (2) <br />• Letters of Commitment (only required if proposing a collaborative project) <br />• W-9 Request for Taxpayer Identification Number and Certification Form <br />• Proof of Insurance (Attachment E) <br />• Indirect Cost Allocation Documentation (if applicable) <br />• Certiflcation and Authorized Signature (Attachment F) <br /> <br /> <br />C. Application Submission <br /> <br />All application documents are provided on the advisory board webpage at <br />https://www.co.kittitas.wa.us/boards/boards.aspx?board=alcohol-substance-abuse. Applicants <br />will email the required documents in PDF format. There is no limit to the number of applications <br />that a single entity may submit for different project proposals.
