REQUEST FOR PROPOSALS FOR THE LIME STABALIZATION AND LAND <br />APPLICATION OF SEPTAGE FROM THE KITTITAS COUNTY SOLID WASTE <br />RYEGRASS FACILITY <br />SCOPE OF WORK <br />1. ln addition to providing all labor, materials, and equipment the Contractor shall <br />perform the following: <br />2. The work is to be done based on the procedures defined in WAC 173-308 and <br />Biosolids Management Guidelines (DOE, July 2000), the application rate will be <br />established by the Department of Ecology. <br />3. The Contractor shall provide daily quantities of septage land applied, lime <br />stabilization, and pH recordings. <br />4. The Contractor shall not exceed permitted gallons as set by the Department of <br />Ecology. <br />5. ln no way is the Contractor allowed to land apply septage in a manner that violates <br />federal, state, or local laws and regulations.6. Contractor shall provide proof of insurance per Exhibit A <br />EXHIBIT "A'' <br />PROOF OF INSURANCE <br />The Contractor shall secure and maintain in effect at all times during performance of the Work <br />such insurance as will protect Contractor, its Support and the Additional lnsured's from all claims, <br />losses, harm, costs, liabilities, damages and expenses arising out of personal injury (including <br />death) or property damage that may result from performance of the work or this Agreement, <br />whether such performance is by Contractor or any of its Support. <br />All insurance shall be issued by companies admitted doing business in the State of Washington <br />and have a rating of A-, Class Vll or better in the most recently published edition of Best's <br />Reports unless otherwise approved by the County. lf an insurer is not admitted, all insurance <br />policies and procedures for issuing the insurance policies must comply with Chapter 48.15 <br />RCW and 284-15 WAC. <br />The Contractor shall provide proof of insurance for: <br />1) Commercial General Liabilitv lnsurance. <br />"*l"Ti:ffi[ffi <br />l][,i,1'1,::l{.Fj:::;::*".sassresa,e. $1,000,000 personaland advertising injury, each offense. Certificate Holder - Kittitas County. The Certificate must name the County as additional insured as defined in the <br />Agreement.
