Professional Services Agreement (rev. 09/24/2018) <br />Page 13 of 17 <br />EXHIBIT "A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following: <br /> <br /> <br /> Removal of approximately 345’sf of asbestos containing tile, mastic, and cove base -All required notifications to Department of Labor & Industries and local clean air agency or -US EPA, where applicable -Mobilization and all necessary labor and equipment to complete the project -Place the area under negative pressure enclosure -Under proper P.P.E. commence abatement -All removed ACM debris will be double 6-mill bag and disposed as HAZARDOUS WASTE -Perform post abatement air clearance -Remove containment and dispose of all hazards at the proper landfill -Provide all completion paperwork <br />This work will be completed after working hours at 205 W 5th ave Ste 105 (Kittitas county Courthouse auditors store room)
