KITTITAS COUNTY <br />Health <br />Network <br />• Work Groups are sub -groups that plan and implement strategies, engage community members <br />and subject matter experts, and track progress towards goals and objectives. Work groups <br />report progress regularly to the Leadership Council and to the Board through the Executive <br />Director. Their purpose is to plan and implement activities which carry out the mission of the <br />Network and the Community Health Improvement Plan. <br />ROLES & RESPONSIBILITIES <br />All Members and Network participants agree to the following: <br />1. Fully participate and contribute positively and constructively to the Network mission and <br />vision, implementation of the strategic plan (Appendix A), and implementation of the <br />Community Health Improvement Plan (Appendix B) <br />2. Commit to practicing the values of community health improvement in Kittitas County (see <br />Appendix C) <br />3. Commit to implementing and participating in the Collective Impact model (Appendix D) <br />4. Commit to centering health equity in the work of KCHN (Appendix E) <br />5. Abide by all meeting guidelines (Appendix F) <br />6. Share new funding opportunities and include the Network and/or Network Members in <br />applications when opportunities align with the Network's mission, vision, goals, strategic <br />plan, and/or the Community Health Improvement Plan <br />7. Engage in and support consensus -based decision making <br />S. Dedicate time for regularly attending Network meetings <br />9. Treat shared information as confidential and agree not to disclose shared information to <br />unauthorized entities <br />10. Disclose any conflict of interest that may arise in the course of Network collaborative <br />activities <br />11. Protect patients and clients and comply with all Health Insurance Portability and <br />Accountability Act and federal requirements <br />BENEFITS OF COLLABORATION <br />Participation as a Member of the Network will enhance population health for our community and <br />strengthen health and social service systems. Additional Member benefits will include the following: <br />1. Cross -sector collaboration through collaborative contracts and grants <br />2. Systems integration through collaborative community health assessment, community health <br />improvement plan, and care coordination <br />3. More equitable communities through collaborative care coordination <br />4. Healthier communities through collaborative information sharing <br />5. Improved cross -sector communication through opportunities for relationship building <br />6. Avoidance of duplication of services <br />7. Leveraging the collaboration for increased access to funding opportunities <br />FUNDING <br />There is no funding specifically provided under this MOU. The Network's annual budget will be <br />approved by the Board of Directors during the annual meeting. <br />www.healthierkittitas.org <br />
