EXHIBIT "A" <br />SCOPE OF WORK <br />CDH intends to provide 24/7/365 access to medically trained personnel to provide <br />prompt access to treatment for Opioid Use Disorder (OUD). Nursing staff will be in the <br />facility daily, with access to prescribers via telephone always. Nursing staff will screen <br />and triage new inmates for Substance Use Disorder (SUD), making appropriate <br />consultation with on -call CDH provider as indicated. Medications for opioid withdrawal <br />will be kept in secure storage at KCJ, ordered utilizing Dr. Deffinbaugh's Drug <br />Enforcement Administration (DEA) license. Nurse case management services will be <br />available to inmates daily, ensuring a smooth transition to continued OUD care upon <br />release from incarceration. <br />Goals and Objectives <br />While there are numerous benefits to be realized from implementation of our proposal, <br />there are three essential objectives that are succinctly summarized below: <br />- Increase access to Medications for Opioid Use Disorder (MOUD) in KCJ. <br />- Reduce risk to inmates by managing opioid withdrawal safely through utilization <br />of buprenorphine in line with current BJA guidelines. <br />- Coordinate care for inmates being released back into the community, reducing <br />recidivism and poor outcomes for individuals with untreated OUD. <br />Proposed Program <br />To accomplish the above -described objectives, CDH intends to hire a 1.0 FTE 40 hour / <br />week nurse case manager (NCM) and a 0.4 FTE 16 hour / week NCM. <br />Buprenorphine will be utilized to manage both ongoing OUD and acute opioid <br />withdrawal for inmates in custody at KCJ. This is in line with current evidence -based <br />guidelines published June 2023, by The Bureau of Justice Assistance. Buprenorphine <br />and Sublocade will be ordered and kept on jail stock at KCJ using Dr. Deffinbaugh's <br />DEA license. Inventory, stocking, and re -order requests will be completed by CDH <br />nursing staff, though may occasionally require a co -signature by KCJ deputies if only <br />one CDH nurse is available. <br />CDH will ensure that they have and maintain the appropriate DEA, state, or federal <br />license's during the length of this contract to order Buprenorphine (Suboxone) and <br />Sublocade and store it at KCJ. CDH will have the sole responsibility for this medication <br />and its storage and use. KCJ will use other funding sources to pay for this medication, <br />but there will be collaboration with CDH to get it ordered appropriately and maintained. <br />Workflow will be as follows: Inmates will be screened for substance use disorder with <br />the Tobacco, Alcohol, Prescription medications, and other Substance Tool (TAPS Tool). <br />This tool is the most currently recommended screening tool according to the National <br />Professional Services Agreement (rev. 09/24/2018) <br />Page 13 of 22 <br />
