Task <br />H <br />Activity Deliverables/Outcomes Due Date/Time Frame Payment Information <br />and/or Amount <br />Subrnit plan to the DOH Pro$am Contact for review and prior <br />approval as soon as possible. We want to be sure your planned <br />activities are allowable, and we will be able to reimburse you for <br />the expenses. <br />Funding is intended to establish, expand, train, and sustain public <br />health staffto support LHJ prevention, preparedness, response, <br />and recovery initiatrves. These include the following short-tenn <br />outcomes: increased hiring ofdiverse public health staff, <br />increased retention ofexisting public health staff, and irnproved <br />workforce systems and processes. Washington will also move <br />toward the following interrnediate outcome measures as part of <br />thrs Workforce initiative: increased size land capabilities] ofthe <br />public health workforce, increased job satisfaction, stronger <br />public health foundational capabilrties, and increased reach of <br />public health services. Ultimately, these workforce inveshnents <br />will support accelerated prevention, preparedness, and response to <br />ernerging threats, and itnproved other public health outcomes. <br />Fr.rnding can be used for pennanent full{ime and part-tilne staff, <br />temporary or term-limited staff, fellows, interns, contractors, and <br />contracted employees. <br />Allowable costs include: <br />o Costs, including wages and benefits, related to recruiting, <br />hiring, and training ofnew or existing public health staff. <br />. Purchase ofsupplies and equiprnent to support the <br />expanded and/or current workforce and any training <br />related to the use of supplies and equiprnent. <br />. Training and education (and related travel) for new and <br />existing staff on topics such as incident management <br />training, health equity issues, working with underserved <br />populations, cultural cotnpetency, disease investigations, <br />informatics or data management, or other needs <br />identified by the LHJ. <br />o Costs ofcontractors and contracted staff. <br />Notes: <br />Preapproval frorn DOH is required to contract with these <br />funds. <br />Preapproval is required for the purchase ofequipment. <br />(Equiprnent is a tangible item with an original per-unit <br />cost of $5.000 or more.) <br />Requests for approval of contracts <br />and/or equiprnent. <br />As needed <br />Exhibit A, Staternent of Work Page 2 of 4 Contract Number CLH3l015-Amendment l7
