Professional Services Agreement <br />Page 13 <br />EXHIBIT "A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following: <br />•Submit a quarterly performance report to the 1/10th of 1% Board Clerk at <br />behavioralhealth@co.kittitas.wa.us : <br />The report shall include, at a minimum: <br />i.A narrative of project activities during the reporting period, including the SMART <br />Goals and Objectives activities outlined in EXlBlT "D" . <br />ii.A narrative of project successes during the reporting period. <br />iii.A narrative of project challenges during the reporting period.
