Comprehensive Healthcare $395,394.76 <br />Comprehensive Healthcare proposes to maintain the two full-time crisis case <br />managers (CCM’s) at KVH. <br />•Crisis case managers will respond in person to crisis and non-crisis <br />behavioral health referrals, provide crisis triage, safety planning and referral <br />services. <br />•CCM’s will assist the ED personnel in contacting the Designated Crisis <br />Responders (DCR). <br />•CCM’s will ensure individuals are connected with a community provider to <br />address any unmet needs. <br />•CCM’s will also provide ongoing behavioral health support to ED staff during <br />the course of providing patient care.
