EXHIBIT "A'' <br />SCOPE OF WORK <br />The Contractor shall provide 100 hours of assistance to the department, to includesome of or all of the following areas: <br />r Public health emergency preparedness and response activities for COVID-lg. <br />This may include surveillance, epidemiology, laboratory capacity, infection <br />control, m itigation, com m unications, commun ity engagement, and/or otherpreparedness and response activities for COVID_19. <br />r collect, organize, analyze, enter, and report covlD-1g data, utilizing CREST, <br />WDRS, and internal tracking sheets. <br />' Work with partners to ensure testing is provided in a culturally and linguistically <br />responsive mannerwith an emphasis on making testing availableio Oisproplortionally <br />impacted communities and as part of the jurisdi-ction's-contact tracing straiegy. <br />r Maintain a current list of entities providing COVID 19 testing and at what volume. <br />' Work with partners to provide culturally and linguistically responsive community <br />outreach and education related to COVID-1g. <br />' Regional active monitoring activities, including partnerships with WA DOH, otherLocal Health Jurisdictions (LHJs) in the - region to collaboratively support <br />epidemiologic and surge capacity needs, and ensure regular cornmunication <br />"rongLHJs in the region. <br />Professional Services Agreement <br />Page 13
