EXH rrAI <br />SCOPE OF WORK <br />The Contractor shall employ one or more Community Health Workers / Promotoras de <br />Salud, who will work with Kittitas County Public Health Department staff as part of the <br />County's vaccination outreach efforts. <br />The COVID-19 Vaccine Outreach Community Health Worker (CHW is a temporary <br />part-time position expected to last approximately twelve (12) months. The employee in <br />this position will specifically perform work in partnership with Kittitas County Public <br />Health and other partners, related to decreasing barriers to accessing COVID-19 <br />vaccination for Kittitas County's Latinx and Spanish-speaking households. The position <br />requires courteous and cooperative interaction with all community partners, County, <br />State, and Federal agencies, and the general public, and will be expected to provide <br />assistance in other areas of Public Health as needed. All employees work under the <br />g u id a nce of loca l/state/federa I reg u lations. <br />IMPORTANT: This is position is expected to end Aprit 26, 2022. Duties and hours may <br />be amended as determined by COVID-I9 funding avaitabitity. <br />Essential Functions: <br />lmprove COVID-19 testing and vaccination access and uptake in local Latinx and <br />Spanish-speaking communities by:. ldentifying and implementing outreach strategies to reduce barriers to testing and <br />vaccination. lncreasing community COVID-19 knowledge. Developing effective relationships with community members and community <br />partners. Assisting with appointment scheduling <br />OTHER FUNCTIONS: <br />. Respond to Public Health related inquiries from the public as needed.o Maintain confidentiality as described in agency, federal, and state required policies <br />This iob description is a summary of the primary functions and expectations for this <br />position.
